Wine in moderation within a Mediterranean diet pattern may do more than we thought—pooled data from over 30,000 people show significantly lower mortality risk when wine is part of the picture versus MedDiet alone. That’s the kind of finding that reframes clinical conversations about alcohol. Plus: time-restricted eating outperformed general diet advice across multiple metabolic outcomes in type 2 diabetes, and new microbiome data from extremely preterm infants suggests early omega-3 supplementation leaves a lasting signature.

Wine + Mediterranean Diet Mortality Signal
A pooled analysis of two Spanish cohorts (n=30,580 combined: PREDIMED trial with 17-year mortality follow-up, plus SUN cohort with 22-year follow-up) found Mediterranean diet adherence with moderate wine intake (≤7 glasses weekly) was associated with 33% lower all-cause mortality compared to MedDiet without wine in PREDIMED (HR 0.67 vs 0.77). CVD risk was 45% lower in the wine-inclusive group.
The benefit disappeared at three or more glasses daily, and some analyses vulnerable to abstainer bias weren’t significant, suggesting the signal is real but context-dependent.
Takeaway: This challenges the blanket “no alcohol” stance by showing wine within a Mediterranean dietary pattern may offer mortality benefits that MedDiet alone doesn’t fully capture—but only at moderate intake levels. It’s a conversation-starter for clients who already drink moderately and resist elimination.

Time-Restricted Eating for Type 2 Diabetes
A network meta-analysis of 13 RCTs (n=805) in overweight or obese adults with type 2 diabetes compared time-restricted eating, continuous energy restriction, and short-term fasting against general diet advice. TRE reduced HbA1c by 0.55 points, body weight by 2.32kg, and improved multiple metabolic markers including fasting glucose and cholesterol.
TRE ranked first across outcomes using SUCRA probability analysis. Direct head-to-head comparisons between TRE and CER weren’t statistically different, meaning both strategies work—and the authors explicitly noted outcomes should be individualized to patient preference.
Takeaway: This gives you evidence-backed flexibility: time-restricted eating and continuous calorie restriction perform similarly for type 2 diabetes management. When a client resists one approach, you have robust data supporting the other—adherence becomes the deciding factor, not superiority of method.

Early Omega-3s Shape Preterm Infant Gut Microbiome
A longitudinal shotgun metagenomics study (n=75 extremely preterm infants) tracked how arachidonic acid (AA) and docosahexaenoic acid (DHA) supplementation shaped the developing gut microbiome and whether those changes contributed to the known retinopathy-protective effect. Supplementation increased Citrobacter koseri abundance and shifted protein and metabolic pathways.
Severe retinopathy of prematurity (ROP) correlated with microbiome diversity measures, but the study couldn’t establish causality between supplementation, microbiome changes, and ROP reduction. Study site and gestational age also influenced microbiome composition, underscoring how many variables shape early gut ecology.
Takeaway: This adds mechanistic depth to why early omega-3 supplementation matters in extremely preterm populations—it’s not just about retinal tissue, but also about setting a microbiome trajectory that may influence multiple systems. It’s exploratory but worth tracking as the preterm nutrition story unfolds.

Some interactions weren’t significant, so interpret carefully—but the dose-response data and pooled signal suggest context matters as much as the compound.
